Hi @alanshortis, what version of the plugin are you running? WordPress version?
What other plugins are you running on your site?
One observation is that your file structure is quite different from a typical WordPress install, can you elaborate on this?

Hello.
Huge apologies for the time wasted on this - after some playing around with the enabling/disabling other plugins, updating everything and entering the credentials for our blob storage it's all working properly again.
The issue started when I changed the account name and key, but I haven't been able to recreate at all.
Just for brevity, I am using v4.0.2 of the plugin in WordPress 4.8 alongside Timber and ACF Pro. The folder structure is as it is as we are obtaining all plugins and WordPress via Composer. We are running a multisite instance and this plugin is enabled automatically via mu-plugins.
I'll be sure to follow up with some additional detail if I see this issue again, but for now I'd say this can be closed.
