Now that caching is implemented, it's much harder to make sense of returning the same event multiple times.

The new rule should be: if noCache is not specified for a filter, give back the same event only once.

Also giving back an older event for a metadata / contacts than the last event that was given back should be disallowed (but that should be turned off by another option).

noCache still needs to give back everything so that a higher layer can use it for checking which relays contain some event.

After a bit more thinking I realized that it's just too hard to think about all the interactions between features, so this feature will stay just simply not giving back the same event twice
