Or have we thought bout decoupling the html templates in general so that they can be customized for any one?

@tlvince @randallmeeker: Good question, one of the top goals of the project is that nothing is required and you can customize everything. HTML templates were baked in from the start with the ability to do this. It's hinted at in the wiki documentation here:

But there is no specific example on the README.md page. Essentially you do this (and I've even done it with BS2 but don't have the code handy):

.config(function (aaFormExtensionsProvider) {
        //custom config as needed

        aaFormExtensionsProvider.defaultFieldGroupStrategy = "somethingdifferent";
        aaFormExtensionsProvider.fieldGroupStrategies.somethingdifferent = function(element) {
            //do something with the element
        };
    })
