I got something working which looks promising:
I wrote a custom OptionsAware type activator that we could register as part of OptionsServices.GetDefaultServices.
The two ways to register options are:
If named options are needed, and what apps typically would use a new constructor attribute:
public MyClass([Options(OptionsName = "name")] MyOptions options, string name = ""]
Frameworks could register their options as part of their default services like this instead:
services.AddInstance(new OptionsInfo { OptionsType = typeof(FrameworkOptions) });

Lou pointed out an issue with TypeActivator, it can only see the most derived constructor so this won't work for any base constructors. We can use the Func for non named options, but there's nothing equivalent for named options.
