Custom registration fields about club-tracker

Things we need for this:

• As a club leader, I need to add fields that need to be asked during registration under each section type: parent, household, child.
• As a parent, I must fill in the additional information needed by the club during registration.
• Persist additional fields required to the database associated with the program.
• Load additional fields required by the program and show them on the registration screen.
• Persist additional field values to the database associated with each person or family.
• Load additional field values and show them when registration is updated.
