I did not do this before. I think this is feasible with SharpAvi depending on incoming frame rate, available computing power and used codec and its settings. For instance, Unfortunately, implementation of MPEG-4 built-in into SharpAvi has limited capabilities to control codec settings (and some codecs do not respect these settings anyway).
I've just checked with the sample screencast app and MJPEG worked flawlessly at 1920x1080@30fps. MPEG-4 encoding with Xvid showed about 15% frame drops on this rate. FYI CPU used was Intel Core i5 2380P. I think with some optimizations and/or more powerful CPU you can get to stable encoding with MPEG-4 as well.

Also there are specialized products for realtime video processing (encoding included) - for instance Medialooks (AFAIK, it uses ffmpeg for encoding/decoding). But they are usually expensive and require powerful hardware anyway.
