My preference would be to throw a useful exception, something like "KeyNotFoundException" instead of the runtime_error. That'd preserve the API's simplicity, ie string x = conn.get("x");

Maybe you could add companion methods for specific operations where boost::optional may be useful, like:
boost::optionalstd::string maybeGet(const std::string& key)

That's my two cents... I've made you a collaborator, so I'll leave the decision up to you - I'm happy to see this fixed whichever way you decide.
