500 normally means php crashed.
Can you post whatever is on the log screen after this happens?

For some reason I cannot attached a .csv file. I've attached a screenshot of the most recent log entries. The last log entry contains "fastcgi://127.0.0.1:9000". I'm not sure if this matters, but port 9000 is already in use on that server by SqueezeBox.

Its having a hard time reading the database, and if it was missing it would have created a new one. I'm guessing something with permissions.
Is this a new install? or was it an upgrade?

On the first screenshot you posted, what does it show for volumes table there?
I am just playing with it now to try to fix this issue, but I think it is with how it was deployed, something with the synology.
