Do you mean people request a new publisher using the form and then there is a manual process to get it added? How about just storing the data in a google spreadsheet that nominated people can edit and improve, wiki-style? Or if you want to moderate, how about storing the data in git and do pull requests?

I like the gdocs approach but spam is a problem. If you were up for booting a gdoc right now for e.g. eu and gb that would be awesome - and then ping okfn-labs and invite contribution :-)

You mean try out gdoc and see if it was better than the form approach?

I think we could have both but right now dumping the CSVs into a public gdoc would be a simple approach for crowdsourcing additions and corrections.

The approach suggested in this issue is to use js to submit stuff via google form to a spreadsheet (what i call the google forms hack and which we use in e.g. https://github.com/okfn/scodaquiz!)
