The "Add Portal" was working in the previous version #1.
Investigated the app and found the issue was caused by previous qt imports upgrade.
Assigned @uabeysinghe to resolve this issue.

NB: In order to be able to test this - and successfully use an enterprise connection - the Tile Package Kreator app must be added as a registered app to the portal. To do so, follow these steps: https://github.com/Esri/TilePackageKreator/wiki/Add-Tile-Package-Kreator-as-a-registered-application-to-ArcGIS-Enterprise
Tested with TPK 1.15.1 on Windows 10 connecting to a portal 10.9.1 (built in authentication) and a 10.8.1 (IWA authentication) - both connect as expected, can upload packages and browse packages. Both portals didn't have suitable basemaps for creating TPK's from out of the box.

Also note, that if I close Tile Package Kreator and reopen, I sometimes see ERR_UNEXPECTED message. go to Settings, choose a different portal and return to the sign in screen (see the successful sign in page), then go to Settings again and choose the desired portal (returning to the expected sign in page).

Testing only successfully completed on Windows on Mac.
On Ubuntu additional certificate installation issues incurred, which prevented testing completion. Not considering this as a showstopper - the 1.5 Ubuntu version will be made available, but no guarantee on its ability to connect to Enterprise can be given. If required, create new issue specifically for Enterprise connectivity on Ubuntu.
