The issue is the user cannot verify the google CPATCHA correctly,
when they try to solve the CPATCHA by clicking on a checkbox, and click on submit button, appears an error that you don't solve the CAPTCHA correctly,
Where all of these occurs in certain time then it gonna fine and after some times the error appears again,
and the error report contains:

PHP Warning: file_get_contents(https://www.google.com/recaptcha/api/siteverify): failed to open stream: HTTP request failed! HTTP/1.1 404 Not Found
in /home/ally/public_html/includes/library/ReCaptcha/RequestMethod/Post.php on line 68

FYI: This issue happened in last 3 days, and until now it doesn't fixed,

And one more question: is there limitation in number of requests per hour?? because I own website for URL Shortener, and I forced the users to solve a Google CAPTCHA before redirect them to a destination page of shorted URL.

Are you getting any timeout error?

If not, this error is possible generated by a wrong POST request or the same request is being sent twice.

wow! am currently having a similar issue. mine is a register form that POST to a user database table. when the register button is clicked it keeps throwing the $errors[] = "Problem with Captcha" message even after checking the checkbox on the reCaptcha images.
