This is what I have so far, it basically matches the example you have for SiteLinks but may require a little house keeping.

First tests seem to work but when I go into Adwords to do an edit I see some unusual errors so may need a little more time on it.

Thanks for the example, but after discussing with the AdWords team, we decided that this is too similar to the existing SiteLinks example so we won't want to add another example for this at the moment. We may rework these examples in the future.
