I didn't implement this, but it's a good question. Maybe @wraithgar can
shed some light.

Yeah it probably should work that way. The way it was done initially was because of its ease of implementation.

+1 on making defaults middleware w/ options as its only parameter (note: will break backwards compatibility of course)

+1 — you could probably support both (at least for a few versions) by just checking to see if an express app is passed versus an options dictionary. If it's an express app either warn but then switch to the new behaviour.
