This can be reproduced in certain conditions, it is highly likely that this is related to #55.

Seems like the second call of DriverAdapter::create() failes. The debugger stopped at opensl_io.c line 356:
notifyThreadLock(p->outlock);
Unfortunately I'm not very familar with C but maybe that helps you. Please let me know if I can support you here. I at least have 2 devices which are making trouble :)

Closing this as it is completely related to a fix done for #55 (OpenSL destruction routine did not nullify outlock)
