### Steps to reproduce
This was reproduced in the source-level Workflow Agent and Agent App tests, using the real control flow with controlled persistence failures.
1. Let an Agent run pause for `ask_human` and create a form.
2. Make persistence of the session snapshot and pending form/tool correlation fail, or supply a paused terminal event without a snapshot.
3. Observe the resulting Workflow pause or Agent App question publication.
4. For Agent App resume, try a missing/WAITING form, or a form that does not match the caller-owned active binding's pending continuation.
Currently the Workflow pause and Agent App question can still be published after persistence fails. The form is then presented even though its continuation is unavailable. On resume, the generator lacks a matching continuation check, and the runner can use `load_or_create` and reach backend run creation without resolved deferred tool results.
The regressions exercise both pause-publication paths, caller-owned conversation/debug-build bindings, and the real missing/WAITING form resolver. Backend calls and application entity initialization are recorded by test doubles; this is not a live Celery/backend reproduction.
### ✔️ Expected Behavior
A user-visible ask_human pause/question requires a saved snapshot and pending form/tool correlation. Agent App resume requires the same caller-owned active continuation and a submitted or timed-out runtime form before beginning a backend continuation run.
### ❌ Actual Behavior
Failed snapshot/correlation persistence can still produce a user-visible pause/question. Missing, waiting, or mismatched continuation state can reach later Agent App resume setup or backend run creation instead of being rejected at the continuation boundary.

<!-- Answer -->
The code at `43af508` matches your description on both pause paths and on the Agent App resume path. This is from reading the code; I did not run any tests.
In the Workflow Agent node, `ask_human` builds the form, then calls `_save_session_snapshot`, then yields the pause event [[1]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/workflow/nodes/agent_v2/agent_node.py#L417-L446). `_save_session_snapshot` catches every exception and only records `session_snapshot_persisted=False` in metadata [[2]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/workflow/nodes/agent_v2/agent_node.py#L714-L749), so the pause is published either way. `save_active_snapshot` also returns silently when the snapshot is `None`, without writing the pending form and tool-call IDs [[3]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/app/apps/agent_app/app_generator.py). That covers your "paused terminal event without a snapshot" case.
In the Agent App, `_pause_for_ask_human` creates the form, calls `_save_session` and ignores its return value, then publishes the question [[4]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/app/apps/agent_app/app_runner.py#L856-L887). `_save_session` returns `False` on failure, so the question is still shown with no resumable continuation [[5]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/app/apps/agent_app/app_runner.py#L1164-L1192). The same `None` snapshot early return applies here.
On resume, `resume_after_form_submission` resolves the draft and agent, then builds the entity and starts the worker. It does not check that the form exists, is submitted or timed out, or matches the binding's pending continuation [[6]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/app/apps/agent_app/app_generator.py#L219-L340). In the runner, `_resolve_pending_ask_human` returns `None` when the form is missing or still waiting, with the comment "run a normal turn, no resume" [[7]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/app/apps/agent_app/app_runner.py#L889-L910). `run()` then calls `load_or_create` and `create_run` regardless [[8]](https://github.com/langgenius/dify/blob/43af5082d3c4ab8a96067370d2f86f214e3c85ae/api/core/app/apps/agent_app/app_runner.py#L667-L687), so a resume with no deferred results reaches backend run creation.
Your expected behavior is the right fix. Treat a falsy `_save_session` result, a `None` snapshot, or an exception from `save_active_snapshot` as a failure of the pause, before the pause event or answer is published. Reject the resume at the generator boundary unless the caller-owned active binding has a pending form and tool-call ID that match a submitted or timed-out runtime form.
I didn't find an open PR that addresses this. [#42140](https://github.com/langgenius/dify/pull/42140) adds a `human_input_required` SSE event inside the same `_pause_for_ask_human`, which would make the unresumable form more visible. It should either wait for this fix or gate its event on the same persistence check.
