Yes, I think your analysis is correct: the module tries to convert field names into nice Lua identifiers, and any extra whitespace at the end will give us an underscore. The fix would simply be to ignore whitespace around field names.

That sounds right. It might be worth ignoring quotes around field names too if you don't already.

That would be the CSV standard, and would be a good thing. I'd then have to be a little more clever about splitting fields, since quoted fields may contain commas. Since CSV is so very common, it would be worth handling this case properly.
