Good point. I think the usual way how to specify in TileMill a different thumb image is the setting of "center" in LatLon?
If it is true (to be verified) and there is a clean pull request implementing this I would be glad to review the code and merge it.

The already existing thumb implementation should be compatible with TileMill and their spotlight preview for Mac OS X - it is the most centred tile . So it is not "randomly" really ;-).

I admit I have a bizarre use of MBTiles :)
What I'm doing at the moment with these files is to publish huge images like here http://madmeg.org/delizie/
However it remains that the arbitrarily chosen tile (ok, not randomly) should be verified to exist?

In general, thumb tile is derived by a defined algorithm - usually on the client. There is no way how the clients should verify existance of a tile before they display it.

We are going to fix the JavaScript Viewer thumb implementation to use the center. Tracked under internal ticket: https://github.com/klokan/tileserver-js/issues/35
