so it might be that you need to do this instead:

 const payload = context.payload?.pull_request || context.payload?.pull_request_target || {};
 return payload.body

actually it's just that in the first commit to a PR, there doesn't seem to be a changes key.

edit again: it's about the fact that there's no changes to show.

Other than edited, this is not the trigger of detect I intended.
If you can't find the changes, you can't find what has changed.
