I didn't test this use case as I don't use nested sub-categories.
I'll add this to my todo list...

After a bit of testing, I understand what's happening.
I've commited a workaround in commit 9944b2a.
With this, the nested categories will appear as traditional categories.
To properly fix this, I'll need to hack the API.
The Tiny Tiny RSS API is not returning nested categories when using the special All feeds, including virtual feeds (-4) cat_id and include_nested enabled for getFeeds.
