Can you elaborate a bit?
The other issues are fixed and will be updated with the next major revision.

Just noticed another typo: the index for D in equations 5.183-185 should be 'x,y' and not just 'x'.

Thanks. Good catch - the differentiation order in d^2 f / dxdy was not defined correctly, and this propagated through to this point. It will be fixed with the next revision of the chapter.
