Are you using OTP-21? Tuple calls are being deprecated and there are some issues being resolved with Mochiweb. Maybe use OTP-20 till those issues are resolved and merged.

I use kerl to switch between versions, works like a charm

@benoitc we have lots of OTP-21 incompatible get_stacktrace calls, also in our dependencies. I didn't have the time to start all the changes and testing with OTP-21 yet.
