There is a function:
public void insertImageAsBase64(Uri imageURI, String alt, String width, String height, Boolean relative, Integer inSampleSize) {

Please have look at how to implement it.

Start with line 231:

also see this function in this file:
private void processShareIntent(Intent intent) {
}
An Image is shared to the app. and a dialog is shown, where the user can set the size and scaling of the image...

b) Why it doesn't work properly on gmail, I don't know, but I can check this...but it takes some time...

What you are looking for is inserHTML:
mEditor.insertHTML("<img src='data:image/png;base64,iVBORw0KGgoAAAANSUhEUgAAAAUAAAAFCAYAAACNbyblAAAAHElEQVQI12P4//8/w38GIAXDIBKE0DHxgljNBAAO9TXL0Y4OHwAAAABJRU5ErkJggg==' />");

I guess embedded images are just blocked:
