Turns out this is an issue with the version of wkhtmltopdf. One needs to use the static version that includes a patched QT, rather than the one from the package manager, from https://wkhtmltopdf.org/downloads.html.
I should clarify this in the README, and maybe add a check for the version to PDF conversion.

I added a check (it tells the user what they have to do to get it to work, what version they need etc, as long as their file contains internal links), plus instructions in the README on how to install wkhtmltopdf properly (with a link to their download page rather than apt-instructions that won't work correctly).
Update to 1.10.0 to get this.
