Annnnnddd, it was my issue.
For reference, it's a path thing. In my case it's a homebrew osx install of node and needing to add
export NODE_PATH="/usr/local/lib/node_modules"
to my ~/.bash_profile
from flightplan.

An alternative solution would be to install it locally and use ./node_modules/.bin/fly <destination>.
