Using @latest produces the same questions and same app directory contents. Apologies for not realizing that I wasn't using the most up-to-date version of create-remix.
And thanks for the explanation about the hidden features! Closing this issue.

try npx create-remix@latest; npm has cached your create-remix so you're stuck using 1.2.3 and not 1.4.0
previous versions of create-remix had hidden features when stacks weren't released yet (app name ending in "-stack" triggered this)
