I would definitely like that, but haven't had a look at it yet. The project you linked is the one I use here, so it might be possible to just use the pageHeader
/pageFooter
id as explained here:
https://github.com/marcbachmann/node-html-pdf#footers-and-headers
i. e., add the following to the top of your markdown file:
<div id="pageHeader">Default header</div>
<div id="pageHeader-first">Header on first page</div>
BTW, I was also thinking of a simpler way to cite and have a list of references at the end of the document... so far I've been manually pushing a numbered list down to the end of the last page like this:
Lorem ipsum... [1].
