Uniqueness of clientHandles

When PATCHing an identity maybe we shouldnt enforce that the new clientHandel is unique? Or at most that it is unique within its scope.

E.g. it should be fine making a client with handle "iPhone" as long as its the only such client on my account. but you having an iPhone client shouldnt stop me from nameing mine like that too.

Add "domain" to a couple of places

A key request should be bound to both a reciver_client_id and receiver_client_domain. Domain should also be used when listing clientIds based upon a handle.

Clarify tags vs handles and allow for querying either

When creating a client tags are optional. In the absence of tags, you can always still find a client via it's parent handle. The lookup API that allows someone to find a set of client_id's should include the ability to specify if you want to search by tag or by handle
