The readme.md says the minimum requirements include the build folder... but hasn't that changed to the dist folder?

Yes, that change after 3.0.1, but it is configurable (now?)

It seems since OCT 2012, the PHP has changed significantly, seems updating requires start from scratch...
Is there a guide to updating from the versions available last Oct to the current?

yeah, it needs a bit of work . Planning on looking updating documentation when I get back from vacation. There's lots there for how to setup from scratch ,

@DavidMacDonald I was wondering if you have some sample pages I could look at to help me write the update guide.

The way I typically develop pages is to have a boiler plate file and a content file. The boiler plate is where I have all the includes for PHP variant, including the include for the content file. So when I updated my site from WET 2.x to WET 3.1 all I did was change the includes to the PHP framework. For me it was just a mass find and replace.

However, I realize that's probably not the way most people are working with the PHP variant and better instructions will be needed for upgrading.
