Quality Testing Form about akrspi

1. Remove Pravesh ki tareeq (we added date of entry for clarity yesterday) it has been added twice, the second time as Pravesh ki tareeq, that needs to be removed.

2. Unique date logic for date ( that it should be different from date of entry) needs to be removed. We mentioned that to convey that there are 3 different date pickers that need to be on the form, not that they need to be unique dates between the 3 fields)

3. Similar to other cards on homepage, cards related to Quality Testing need to be added.
