Provide option to disable default mappings

Nice plugin! It would however be nice if there were a setting that disabled the default mappings, something like g:textobj_markdown_no_default_key_mappings that defaulted to 0 but could be set to 1.

Enable selecting code fence without language

The text objects for selecting a code fence don't work if the code block has no
language set. Like

```
A = B + C
C = D + E
```

Does this plugin provide an option to change that behavior?
