Change Title

Change title from "Coronavirus Questionnaire" to "COVID-19 Check"

Add Data Explainer

Add Explanation on our Data Protection Policy in the landing page(with a modal pop-up) with the following text:

What data is captured?

The following information is collected for each submission: gender, age, symptoms, duration of symptoms in the case of red-flag symptoms (such as fever, cough and shortness of breath), temperature, user intent (that is whether they want to test for themselves, their family or just to try it out).

There is also metadata about the submission, such as entry id, session id, scenario (the final advice that was given) and at what time has the submission been created (created_at).

How is the data captured?

Each submission of data is captured by a web-based application built using Node.js and stored in a Postgres database.

Where is the data stored?

The Postgres database is located on an Azure Virtual Machine managed by Malta Information Technology Agency.

How long is the data stored for?

The data (which is all anonymous) is expected to be retained for the duration of the COVID-19 crisis and one year thereafter for research purposes.

Why is the COVID-19 Public Health Response Team doing this?

The Team is asking for these data as part of its efforts to track the spread of symptoms that may be related to COVID-19 infection, as part of its nationwide syndromic surveillance efforts. They will help the Team in its disease prevention and control activities.

Remove Text from Welcome Message

Since we won't be tracking IPs, I would remove the following text: "By submitting this questionnaire, I agree to my IP address being logged for the Maltese Superintendence of Public Health to differentiate between multiple entries. Your IP will not be used to identify you."

Change Text in Welcome Page

Change text from:

"This short questionnaire will help assess your risk of being infected and provides recommendations based on your situation.

Your answers in turn help the Maltese Superintendence of Public Health assess and predict the spread of the virus in Malta.

By answering the survey, you will receive further suggestions on how to behave in your situation."

Remove 'Share Location' Question

There is little to no value with sharing a snapshot of location with respect to Public Health, so I would recommend removing this aspect.
