Add Outreachy application template

We should add an outreachy proposal template as well as a GSoC one.

Remove the GSoC logo and re-brand to be outreachy where appropriate. Let us know if you think anything else needs to be changed. Once it is done, share with a mentor and/or @yochannah and make a PR to update http://intermine.org/internships/guidance/students-applying-outreachy/ with a link to the new PR (code is here: https://github.com/intermine/internships/blob/master/guidance/students-applying-outreachy.md )

GSoC proposal template

My suggestions:

1. no page limit on the proposal part. (cloud one is going to be LONG)
2. add a section for risk analysis. Make optional as maybe not appropriate for all?
3. example proposal for students? Nupur and Kostas agreed to let us use theirs

Each of my project specifications has a (broken) link to the proposal. Let me know when this is done and I will update with the good link.
