Add a plugin customization dialog

The plugin customization dialog should also be accessible from the main Calibre GUI preferences -> plugins dialog.

Example for settings items:

  • customize QR text currently, something like "Completed {book_title} - {chapter_title}"
  • Activate alt text cues for accessibility
  • Heuristics behavior (enforce, no report, etc.)

Add a bug report mechanism within the plugin

Add menu and toolbar entries

  • About this plugin
  • Report a bug

Ideally it should be in the form of a wizard, and should be callable from the various error dialogs.
With questions as

  • Is it a misbehavior or a feature request?
  • What action were you performing (auto-fill accordingly)?
  • Include version and backtrace info (autofilled)
  • optionally, describe if document seems to have particular quirks (unclosed tags, etc.)
