There should be mechanism for async resolution, based on getaddrinfo_a. The examples are: Kazuho Oku; Stack Overflow. See also: you can longjmp() out of signal handlers.

Backend connections should be decoupled from frontend connections, since on one keep-alive frontend connection there can be requests to different backends. Backend connections after finishing the response should be put into backend pool from where they can be retrieved later by peer IP. They should remain in pool until they retrieved for serving next request or remote end closes the connection or they expire by local timeout.

There should be 4 buffers instead of 2: 2 for passing requests and 2 for passing responses.

Host header value as well as IP-address must be copied to preserved buffer and compared against Host header in next request. If next request's Host is different, then it must be resolved and its IP compared to previous Host IP. If IP-s are different, then existing backend connection must be shut down and the new one established.
