Improve error handling/displaying

Currently if something goes wrong in the login process, a user just gets a blank white screen with no explanation of what's happened.

I've had this happen when waiting for a while on the Duo screen logging in via LDAP - I assume it's just down to the length of time taken to log in, so should be able to be replicated with passwordless too.

The errors I get in logs:

(auth0) Authentication failure! invalid_credentials: OAuth2::Error, invalid_grant: Invalid authorization code
{"error":"invalid_grant","error_description":"Invalid authorization code"}

NoMethodError (undefined method `error_reason' for #<OAuth2::Error:0x007f46324eda48>)
