A welcome page, include the README, etc
Manually added CNAME as this gets wiped out with the use of gh pages (original) but is retained when using GitHub Actions (beta). However, quarto doesn't publish to gh-pages with GitHub Actions (beta).

The placeholder pages "rural" and "Summary" have no content. We should remove them until they are needed.
It might be worth dropping the "QuartoBook" from the name as this describes how it's been built rather than what it contains. Perhaps then expanding on HIQ to health-inqualities-notebook
or something similar?

Some things are flagged in the local system GPRCC/Ehealthscope (a Nottingham based system like a TRE which is available for GPs to access for patient level care and now hosts public health data like cause of death for public health analysis only).

It would be great to add a README when time allows, I was just thinking about showing somebody but there isn't anything to link to yet
For example:
Cover high-level categories, and detail.
Cover differences between NHS and ONS categorisations (if they exist) - keeping to primary sources eg. NHS data dictionary.
Consider the importance of standardised rates in reporting and assessing health inequalities, differences between crude rates, direct and indirect standardisation. Technical approach to generating standardised rates.
Delete folder like _book as recommended by https://quarto.org/docs/publishing/github-pages.html#ignoring-output.
As this is now published through the docker container in GitHub there is no longer any need for the gh-pages or the _book folder as anything merged will be automatically run through the gh-actions.
Including how to deal with trans and intersex patient data.
This book should be published to github pages. I'm happy to arrange that but don't currently have permissions. Might be worth dealing with issue #1 first.
