Minimal asciidoctor support

We shall at least support the bare minimum asciidoc syntax before publishing ascii-nure on crates.io.

zero copy parsing

The parser currently copy each input slice into corresponding Elements content. We shall instead use reference to input indexes, and copy the relevant input slice when generating html output.
