Bug when using Hot Module Replacement

I'm not sure if this is an issue with conditioner or a webpack settings issue.

When saving an inner module it seems to not get the correct path when reloading. It seems to add a new ./ui and .js so conditioner can't find the module.

Media condition gives errors

I'm getting the message "Cannot find module '././monitors/media'" when using a media condition.

WebPack 5 support

Hi @rikschennink ,

I'm having some issues trying to make this work with WebPack 5. I see is this boilerplate project was created for WebPack 2. Is it supposed to work with WebPack 5 too?
