Create Well Structured README file

As of now, the README file doesn't have enough descriptions to make this Ultimate Resource handy. So might be we can structure a Readme file according to subject name along with subject code that can direct a person to the subject folder. Then I guess we can find our desire subject resource more quickly.

So, What might be the best way to structure the readme file? so that we can work upon that...
