Reddit Medical Sycophancy — Benchmark + Evaluation Pipeline

A benchmark for measuring sycophancy in natural medical scenarios — and the infrastructure to test what induces it and how to mitigate it.

Project goals

  1. Build a benchmark. 5,241 real r/AskDocs posts where a physician explicitly corrected a user's wrong belief, screened to a clean test set. This is the substrate — a reusable way to measure whether an LLM endorses a wrong medical belief in a realistic conversation.
  2. Identify what induces sycophancy. Run models under different system prompts (empathy-first is the headline example, but not the only one) and quantify which framings push models toward agreeing with wrong beliefs.
  3. Reduce sycophancy. Use the benchmark to evaluate mitigation strategies — prompt engineering, fine-tuning, decoding-time interventions, and other approaches.

The pipeline below is the current implementation of (1) and (2). Mitigation work (3) plugs in at the model layer: any model or prompt that runs through test_vllm_models.py gets scored against the benchmark.

Data: 5,241 r/AskDocs posts where a physician explicitly corrected a user's wrong belief, screened from a larger Reddit corpus via Arctic Shift.

Quick start (local, OpenAI models)

System prompts (the lever for studying inducers + mitigations)

Each model is run once per system prompt against the same benchmark. New prompts go in pipeline_config.py under SYSTEM_PROMPTS — no code changes needed.

Inducers to test next (open question, not yet implemented): authority-deference prompts, user-confidence priming, role-play personas, etc.

Mitigations to test next: explicit anti-sycophancy instructions, chain-of-thought self-critique, ensemble agreement, and (separately) fine-tuned variants of the benchmark models.
