Physical MFA Devices Unsupported

Using a non-virtual MFA device is not supported:

An error occurred (AccessDenied) when calling the GetSessionToken operation: MultiFactorAuthentication failed, unable to validate MFA code. Please verify your MFA serial number is valid and associated with this user.

It looks like the code assumes a virtual MFA device, using the username from the CLI to construct the string that refers to the virtual device. It would be great if physical keys were also supported. Otherwise, it's impossible to enforce MFA for the entire account as long as there's at least one user with a physical key.
