Can I customize validation error handling? For example, to return response in json format, if validation failed.

I was getting an error validating the id:
Invalid schema content: (id.$_root.alternatives)

Initial research suggested it was due to a deprecated version of Joi ( "joi": "10.6.0") being used in koa-joi-validate.

I got it working by creating a validate.js locally and changing
const { error, value } = joi.validate(object, schema, options);
to
const { error, value } = joi.object(object).validate();

in the koa-joi-validate index.js

Not sure if it's robust, but may help others.
