Index does not work on Github.
How to get around this problem?

Some markdown documents use the level 1 heading as document title and levels 2 and above for section headings so a configuration setting for the minimum level as well as maximum would allow for the first level headings to be excluded.

There is possibility to show table of content of subpages.

Would be nice if it also work with that extension

I have some Markdown README's with headings containing . and even { & }. The links generated from this extension are invalid for such cases. The fix would be to remove these special characters such that a heading like # {MAJOR} gets translated to [some_link](#major), and a heading like ## v2.3.4.2f589e85 gets translated to [some_other_link](#v2342f589e85).

The text that's passed to the table of contents includes any markdown tokens, so any additional formatting in headers results in results similar to this:

The markdown syntax supported by the Wiki feature of Azure DevOps uses [[_TOC_]] to generate the table of contacts, so the extension should support this as well.
