Recently, the President announced the COVID vaccination attestation mandate for employers and the impact to the public sector and the private sector. This solution provides organizations with a PowerApp to deploy in response to the mandate.

Vaccine Card Collection

We want to collect vaccine card images using the Dataverse table, not SharePoint. How do we add the field to collect the vaccine image in the 'Submit My Attestation' form? By default, the field is not there and we're unable to find instructions on how in the Deployment Guide.

Permissions for Attestation table

'Manage Permissions' setting in Attestation table is not allowing me to set the permission to 'private' - it continues to reset to the default setting of 'none' and as such users are unable to see their own attestation submissions under the "My Submissions" screen. Please advise. TY.

What o365 group guid do we use in Step 4

hello,

I have reached the section where I'm inputting the admin group guid. I was wondering which o365 group I should be putting here? as well, it appears that the name field is required -- is there a specific name we have to input?

Configure Application Text Not Working

I have installed this app as per the deployment guide. When I input text into the fields in the Configure Solution Settings screen (i.e. Configure Application Text), none of the text that I input appears on the attestation form. See the screenshots below.

I was able to successfully update the other configuration items including: Work Locations, Attestation Statuses, and Useful Linls.

Invalid name for attribute publishedby

Receiving this error 5 times when I attempt to import the solution:

Invalid name for attribute publishedby. Custom attribute names must start with a valid customization prefix. The prefix for a solution component should match the prefix that is specified for the publisher of the solution.

Having issues importing

When attempting to Import the zip file, I am getting an error. The message in the error log says, "The entity relationship role of the referencing entity is required when creating a new one-to-many entity relationship lk_cr8f3_attestationvachold_createdby."

Error importing app package into PowerApps

I am receiving the following error when trying to import the zip package. If I cannot get passed this it is a show stopper for us.

Drop down issue with attestation status -certification attestation

The version of the VAX App that downloaded for me has different details than the one in the guide. When setting up the attestation status and if choosing fully vaccinated, it is asking to enter date of last COVID test, not asking for date of last vaccine. How do we change this? We were able to revise the verbiage a little bit in the tables but the verbiage does not carry over into the member's viewing of their submissions and would also like this to carry over into the reporting pulled into Excel.

Exporting submission results

I am having an issue or more of a question with submission results hoping you can clarify or help with here. I know how to export results from the attestation table in the CDS database but that uses excel and does not provide the capability to view images of the vaccine cards.

I have also used the WiKi page documentation and used the flows to setup the Vaccine Images Library in SharePoint and that works. I had an issue to start with that where the flows were running but not showing up. Might be worth it put in the documentation you need to manually enable the flow once its setup.

The issue I am having is this. If i export from the CDS database i view the content of answers submitted by the user but not images. If I use SharePoint images library for storage, I can see images but not the answers to the questions submitted. I can only view the images. Is there are way that I can view both the images and answers submitted in the same place?
